Illness

• Allergies: Allergic reactions, seasonal allergies
• Ears: Acute otitis media, swimmers ear, Ear drum rupture, Vertigo
• Eyes: Conjunctivitis, Stye
• Nose: Acute sinusitis, allergic rhinitis, vasomotor rhinitis, post-nasal drip
• Throat:Tonsillitis, strep throat, pharyngitis
• Gastrointestinal issues (GI): Stomach flu, food poisoning, and other gastrointestinal issues
• Respiratory infections: Upper respiratory infections, bronchitis, and pneumonia
• Skin: Rashes, minor burns, and other skin irritations
• Urinary: UTI, Kidney Stones
• STD Testing: Assessment and Treatment of symptoms

Medication Refill

Medications can only be filled if the following apply

• Must have prescription bottle or print from pharmacy
• Must have not been filled at our facility before
• Must have been actively taking in the last 30 days
• Medication is not a narcotic or psychiatric medication

Exceptions have been made previously to traveling and displaced patient populations. 

Workers Comp

Our facility at this time does not see chronic condition worker's comp cases. 

New Patients

Requirements to Schedule 

• First Report of injury
• Workers Comp Company
• Policy Number
• Point of Contact Phone, Fax or Email
• Claim Number required for visits that occurred greater than 24 hours ago
• Employer Name, Address, Point of Contact, Phone, Fax or Email

New Patients Seen Elsewhere

Must also Bring

• Previous facility records or discharge notes
• Final results of any imaging.
