Medication Refill

Medications Can Only Be filled if the Following Apply

  • Must Have Prescription Bottle or Print from Pharmacy
  • Must have not been filled at our facility before
  • Must have been actively taking in the last 30 days
  • Medication is not a narcotic or psychiatric medication

Exceptions have been made previously to traveling and displaced patient populations. 

School and Sport Physicals

For patients 3-18 Years Old

  • School Physical Requires Immunization Record for Evaluation

Workers Comp

Our facility at this time does not see chronic condition worker's comp cases. Additionally, you must be seen within 30 days of your initial injury or last Worker's Comp Evaluation.

Requirements to Schedule

  • Workers Comp Company
  • Policy Number
  • Point of Contact Phone, Fax or Email
  • Claim Number required for visits that occurred greater than 24 hours ago
  • Employer Name, Address, Point of Contact, Phone, Fax or Email

Must also Bring

  • Previous facility records or discharge notes
  • Final results of any imaging.

DOT Physical

CALL OUR FACILITY TO ENSURE A DOT CERTIFIED PROVIDER IS ON DUTY

There are restrictions to DOT certification.

If you have any of the following conditions please make sure to bring all required documentation to expedite your certifications.
