DOT

CALL OUR FACILITY TO ENSURE A DOT CERTIFIED PROVIDER IS ON DUTY

There are restrictions to DOT certification.

If you have any of the following conditions please make sure to bring all required documentation to expedite your certifications. 

  • Diabetes
  • High Blood Pressure
  • Epilepsy/Seizures
  • Heart Disease
  • Sleep Apnea
  • Recent Surgery Last Month
  • Respiratory Disease
  • Vision Problems

DOT certification includes a basic UA to assess for glucose, protein, and blood in your urine. 

Injury

  • Animal Bite
  • Back pain
  • Broken bones (closed fractures)
  • Bumps & Bruises
  • Burns
  • Cuts & Scrapes
  • Foreign Body Removal
  • Lacerations
  • Minor Surgical Procedures
  • Other injuries that are non-life threatening
  • Rashes
  • Sports injuries
  • Sprains & Strains

Illness

  • Allergies: Allergic reactions, seasonal allergies
  • Ears: Acute otitis media, swimmers ear, Ear drum rupture, Vertigo
  • Eyes: Conjunctivitis, Stye, etc.
  • Nose: Acute sinusitis, allergic rhinitis, vasomotor rhinitis, post-nasal drip
  • Throat:Tonsillisits, strep throat, pharyngitis
  • Gastrointestinal issues (GI): Stomach flu, food poisoning, and other gastrointestinal issues 
  • Respiratory infections: Upper respiratory infections, bronchitis, and pneumonia 
  • Skin: Rashes, minor burns, and other skin irritations 
  • Urinary: UTI, Kidney Stones
  • STD Testing: Assessment and Treatment of symptoms

Medication Refill

Medications Can Only Be filled if the Following Apply

Exceptions have been made previously to traveling and displaced patient populations. 

  • Must Have Prescription Bottle or Print from Pharmacy
  • Must have not been filled at our facility before
  • Must have been actively taking in the last 30 days
  • Medication is not a narcotic or psychiatric medication

School and Sport Physicals

For patients 3-18 Years Old

  • School Physical Requires Immunization Record for Evaluation
  • Sport Physicial

Workers Comp

Our facility at this time does not see chronic condition worker's comp cases. Additionally, you must be seen within 30 days of your initial injury or last Worker's Comp Evaluation. 

New Patients

  • Requirements to Schedule: First Report of injury, Adjuster Name, Adjuster Phone, Adjuster Fax, Worker's Comp Company, and Claim Number

New Patients Seen Elsewhere

  • Must also Bring: Previous facility records and imaging
